Communication errors.
According to multiple studies from the NIH and other healthcare research groups, it is the number one cause of medical errors with patients in the United States.
Also, according to multiple studies from that same organization and others, it is possible that medical errors could be the third leading cause of death.
Much work has been done over the years to reduce medical errors through improved quality, checks and balances, but communication errors are still rampant across the industry.
Multiple thought leaders in space point out that many healthcare workers are afraid to speak up because of fear of retribution or loss of their jobs as a key contributing factor.
Why is that the case? And what are the organizational cultural conditions that create such an environment of fear?
